- COVID ASSESSMENT FORM
Print and complete a COVID-19 ASSESSMENT FORM no more than 24 hours prior to the event and bring it with you

- The event is running rain or shine!
- COVID-19 assessment forms are now available. Please download, print and complete the form no more than 24 hours prior to the event. Bring the form with you to submit when you arrive at the event.
- Please come prepared to wear an appropriate face covering whenever you are not seated and stationary in your separated family “bubble”. Guidance from various authorities is changing and varies, but please keep in mind that this event will see a mix of fully vaccinated, half vaccinated and unvaccinated attendees. Let’s be safe and respectful of all! Please have your face mask with you and use it in any situations where separation cannot be maintained or if you go indoors.
- The Service is scheduled to start at 4pm. Please arrive early leaving ample time to park, check in, turn in COVID assessment form etc…

COVID-19 Information
Prince of Peace is committed to following all COVID-19 safety guidelines as required by the Province of Ontario and the Region of Halton at the time of the event. We require that all of our guests follow these same guidelines.

Parking
Parking is available OFF SITE at the Mount Royal Plaza (north of the church, on the same side of Brant Street) and the Burlington Power Center (south of the church, on the opposite side of Brant St. – Sobey’s, Marshalls, Indigo lot)
If you are able, consider proceeding directly to one of the designated parking areas and walking to the church.
Site Layout
When entering off of Brant St. you will be able to drop off passengers at the front of the church and make your way to off site parking. Our parking lot is closed to vehicles, except if you have an accessible parking space assigned in which case you will be directed where to park your vehicle.
You must visit the registration table to check in and drop off your signed COVID declaration form for those in your “bubble”. If you did not fill out a form in advance you will be required to do so before entering.
The Service will take place in the back parking lot and grass area. Seating will be designated into “bubbles” based on your registration.
Please follow directions from ushers and other volunteers when you arrive.
